Athletic Forms 26-27

Pre-Participation Packet (2026-2027): Students planning to participate in athletics must complete the SDHSAA Pre-Participation Form Packet, including a physical completed by a healthcare provider. Paper copies are also available in the high school office. All required forms must be on file with the Activities Director before students can begin practice. Download the packet here.

Concussion Test (2026-2027): Students planning to participate in athletics must also complete a baseline concussion test before participating in practices. Testing dates and times can be arranged with the athlete’s coach.
